California Code, Title 22, Division 6, Article 8, Section 80092 and following sections delineate the health conditions that are allowed in community care homes and the rules for providing needed care. Community Care Licensing (CCL) requires that the care home administrator submit an addendum to their program design for every restricted health condition they are willing to serve. Once approved, then when an individual consumer is admitted to the home, the staff needs to be trained by a licensed health professional and a RHCCP developed for the condition.
VMRC has developed some templates for addendums for all restricted health conditions and a template for a RHCCP. They are here in both PDF (read only) format and in a Word format, which can be typed into. Care providers are free to use these templates, but must make the document personal to their care home and/or consumers.
